The links below will enable you to download, print, complete and sign all of the necessary enrollment agreement, waiver and medical history/exam forms prior to signing up for any Adventure Scuba Company course.

The forms are in Adobe Acrobat Portable Document Format (PDF). To see and print them, you will need Adobe Acrobat Reader software. If for some (difficult to imagine) reason you do not already have this software installed on your computer, you can, of course, download it free of charge from Adobe's website.

• NAUI Waiver/Release Form for Non-Technical Courses (2 pages, 28k).
• NAUI Waiver/Release Form for Technical Courses (2 pages, 32k).
